We specialise in palliative care, rehabilitation, aged and dementia care, mental health services for older people and other related health and aged care services. HammondCare seeks to embed evidence-based best practices in its services and we provide these services through home care, sub-acute hospitals and residential aged care.
Bringing these health, hospital and aged care services together, HammondCare has been able to develop innovative, flexible care models designed to serve people with complex health or aged care needs, regardless of their circumstances.

About the role
Located primarily in the Northern Beaches Specialist Community Palliative Care Team, this permanent part-time 3 days per week role provides palliative care in-reach into residential facilities to provide assessment, care-planning and review.
About you
- Current registration with the Australian Health Practitioner Regulation Agency
- 4 years post-graduate experience. Has or working towards a post-graduate palliative care or related qualification.
- Able to work autonomously within the wider multidisciplinary team.
- Effective communication and interpersonal skills
- Organisation, networking and time management skills
- Able to work with sensitivity, empathy, and flexibility.
- A current driver’s license and safe driving record.
- Willingness to travel between HammondCare’s sites and services.
